This form is required for all minors to participate in any off-campus activities and must be notarized. Our administrative staff is available to provide notary services Monday - Thursday during office hours or Sundays at the Information Center.

Keep your address and contact information current with this form which may be submitted anytime during the year.
Event Request Form
This form is required to schedule any ministry supported events on or off campus. Fill out and submit the form to Cliff Troxell for approval.
