This form is required for all minors to participate in any off-campus activities and must be notarized. Our administrative staff are available to provide notary services Monday - Friday during office hours.
Keep your address and contact information current with this form which may be submitted anytime during the year.
This form is required to schedule any ministry supported events on or off campus. Fill out and submit the form to Cliff Troxell for approval.
